About the Job

Location: Remote – USA

Overview

We are seeking a full-time FHIR Integration Engineer who will serve as the sole owner and technical lead for all EMR integrations across our organization. This role is critical to the success of our interoperability and data access strategy, with full responsibility for designing, building, and maintaining secure, scalable integrations with EPIC, Cerner, MediTech, and other electronic medical record systems. The ideal candidate will bring deep knowledge of FHIR, HL7, and X12 standards, strong technical skills in API development and data transformation, and the ability to independently manage the entire lifecycle of clinical and administrative data exchange.

Key Responsibilities

  • Own and manage all EMR integrations across the organization, acting as the single point of accountability for planning, execution, and support.
  • Lead the end-to-end implementation of FHIR and EMR-specific API integrations with EPIC, Cerner, MediTech, and other major platforms.
  • Install, configure, and maintain Rhapsody Integration Engine environments (test and production) to support HL7, X12, and FHIR interfaces.
  • Design, develop, and monitor data exchange workflows using: HL7 v2 messages (ADT, ORU, SIU), X12 transactions (270/271, 837/835), and FHIR API interactions (R4 and beyond).
  • Collaborate with EMR vendor contacts (e.g., EPIC TSMs, App Orchard, Cerner Ignite) to ensure compliance and implementation alignment.
  • Develop and maintain cloud-based ETL pipelines using platforms such as Azure Data Factory, AWS Glue, or similar.
  • Utilize SQL, Python, or C# to transform, validate, and troubleshoot clinical and billing data payloads.
  • Produce robust technical documentation, internal libraries, and reusable integration patterns to support data consumers.
  • Proactively identify integration bottlenecks, data quality issues, or API performance concerns—and implement solutions.

Required Qualifications

  • 5+ years of experience in health IT integration or healthcare data engineering.
  • Deep expertise in: FHIR (R4) and RESTful API design, HL7 v2.x (ADT, ORU, SIU), X12 EDI (270/271, 837/835), OAuth2 workflows and token-based authentication.
  • Proven hands-on experience integrating with EPIC, Cerner, and/or MediTech EMRs.
  • Proficiency with: SQL and relational database systems, Scripting/programming in Python, C#, or equivalent, Cloud ETL tools (Azure Data Factory, AWS Glue, Informatica Cloud, Talend).
  • Experience installing, configuring, and supporting Rhapsody Integration Engine.
  • Strong documentation, organizational, and communication skills.
  • Ability to independently lead complex projects with minimal supervision.

Preferred Qualifications

  • Experience with SMART on FHIR, Bulk FHIR, or CDS Hooks.
  • Familiarity with revenue cycle workflows such as eligibility verification, prior authorization, and claims processing.
  • Understanding of HIPAA compliance and secure data exchange practices.
  • Exposure to CMS interoperability mandates.
  • Familiarity with NCPDP is a plus, but not required.
